Primary Care Clinician (PCC) Plan Quarterly
newsletter [Current]
[Archived] informs providers about the latest policy updates and
contains articles about medical and behavioral health integration issues. The
newsletter includes stakeholder viewpoint columns from consumers, providers,
trade associations, and public agencies as well as clinical information on best
practices and current research from journals.
PCC Plan Health Education Materials Catalog provides PCCs with current
health education materials for clinicians and PCC Plan Members.

Facilities use this form to notify MBHP
of an incident.
Medical Management Monitoring Form(3M Form):
To promote preventative care and illness management among MBHP Members, each
Community Support Program (CSP) is requested to complete the Medical Management
Monitoring (3M) form with each Member receiving CSP services at the beginning
of the case and at the time of discharge from CSP services. MBHP Members are
asked to answer questions regarding their utilization of emergency room visits,
PCC appointments, Pap Smears, and mammograms to promote their awareness of and
compliance with preventative care. To measure for appropriate illness
management, Members are also requested to answer questions about chronic
illnesses such as asthma and diabetes with particular attention given to
education about medication and treatment compliance. These completed Medical
Management Monitoring forms can be faxed to the MBHP Assessment Unit at
617-790-4129 or called in to an assigned care manager at MBHP.

Communication between Behavioral Health Providers and Primary Care Clinicians "Two Way" Communication Form
The Primary Care Clinician (PCC) Plan and the Massachusetts Behavioral Health Partnership,
along with Beacon Health Strategies, Fallon Community Health Plan, BMC HealthNet Plan,
Network Health, and Neighborhood Health Plan, have collaborated to create a single form that
can be used by all MassHealth providers to facilitate communication. Behavioral health
providers and PCCs can use the form when communicating with one another about a MassHealth
Member.
The primary purpose of this quality improvement initiative is to increase the frequency and
the quality of the content of communication between behavioral health providers and primary
care clinicians. Click here
