This training is meant for community health workers,
health outreach workers, health educators, promotoras, patient navigators, case managers, and other staff who function as liaisons

Webinar: An Integrated Approach to Treating Depression in the Adult Primary Care Setting
In this Webinar, a team comprised of a primary care-based behavioral health clinician, primary care physician, and psychiatrist presented an overview of evidence-based approaches to treating depression in adult primary care. Presenters discussed collaborative care, the IMPACT/Diamond model, and the Behavioral Health Consultant model. They also explored the roles of an integrated treatment team (behavioral health clinician, primary care clinician, care manager, and consulting psychiatrist).

October 2013 Webinar on Motivational Interviewing
Motivational Interviewing (MI) is a collaborative, person-centered approach to facilitate behavioral and lifestyle change. In a 90-minute Webinar, Dr. Judith Bayog,
licensed psychologist and clinic director of the Alcohol & Drug Treatment Program Outpatient Clinic at the Brockton Division of the VA Boston Healthcare System, provided an overview
of MI designed for primary care clinicians and behavioral health providers. Dr. Bayog shared the basic principles and key concepts of MI that help to establish rapport, engage and retain
clients in treatment, and guide them toward change. In addition, Dr. Bayog reviewed applications of MI across cultures and settings.

PCC Plan Health Education Materials Catalog provides PCCs with current
health education materials for clinicians and PCC Plan Members.

Facilities use this form to notify MBHP
of an incident.
Medical Management Monitoring Form(3M Form):
To promote preventative care and illness management among MBHP Members, each
Community Support Program (CSP) is requested to complete the Medical Management
Monitoring (3M) form with each Member receiving CSP services at the beginning
of the case and at the time of discharge from CSP services. MBHP Members are
asked to answer questions regarding their utilization of emergency room visits,
PCC appointments, Pap Smears, and mammograms to promote their awareness of and
compliance with preventative care. To measure for appropriate illness
management, Members are also requested to answer questions about chronic
illnesses such as asthma and diabetes with particular attention given to
education about medication and treatment compliance. These completed Medical
Management Monitoring forms can be faxed to the MBHP Assessment Unit at
617-790-4129 or called in to an assigned care manager at MBHP.
Provider Connection, formerly named the "Primary Care Clinician (PCC) Plan Quarterly,"
is an integrated newsletter featuring news, resources, and information benefitting both PCCs and behavioral health providers.
Provider Connection contains stakeholder viewpoint columns, policy updates from MassHealth, and tips to increase practice
efficiency and tackle common challenges. It also functions as a forum for providers of all types and sizes to share best practices
and research ideas. Additionally, Provider Connection offers listings of events and trainings relevant to both
PCCs and behavioral health providers.[Current]
Health and Wellness Library
MBHP has an online Health and Wellness Library on a website called Achieve Solutions.
This health library is a resource for both providers and Members.
For providers, the Health and Wellness library offers "one-pager" health tip sheets on a wide variety of health conditions that can be printed and given to Members;
links to local resources; and content that can help patients handle difficult-to-address concerns such as bullying.
Providers can direct Members to the Health and Wellness Library to promote self-management of health conditions and
adherence to treatment plans. Clinical content in the library is evidence-based, written by subject matter experts, and
reviewed on an annual basis. Just a few of the library topics include:
In addition to regular content, each month highlights a different health topic. Examples include sleep, emotional and physical abuse, and living with disabilities.
- Healthy weight (BMI) maintenance
- Smoking and tobacco use cessation
- Encouraging physical activity
- Healthy eating
- Managing stress

- Identifying psychiatric symptoms through self-assessment

Through the Health and Wellness Library, Members will be able to access comprehensive, integrated information related to physical and behavioral health, wellness, and more. The site includes interactive quizzes, trainings, articles, videos, web links, and other tools. The Health and Wellness Library encourages Members to become active participants in their health care, and to make healthy choices by providing resources and tools to do so.

Communication between Behavioral Health Providers and Primary Care Clinicians "Two Way" Communication Form
The Primary Care Clinician (PCC) Plan and the Massachusetts Behavioral Health Partnership,
along with Beacon Health Strategies, Fallon Community Health Plan, BMC HealthNet Plan,
Network Health, and Neighborhood Health Plan, have collaborated to create a single form that
can be used by all MassHealth providers to facilitate communication. Behavioral health
providers and PCCs can use the form when communicating with one another about a MassHealth
