Adolescent Treatment Outcomes Module (ATOM)
This is a web based module that is designed to measure outcomes for adolescents
in clinical care settings.
Behavioral and Emotional Rating Scale (BERS)
The BERS assesses the behavioral and emotional strengths of children and is designed for use in school settings.
Behavior and Symptom Identification Scale (BASIS)
The BASIS is for administration with mental health patients aged 14 and older. It provides patient profiles and measures change in self-reported symptom and problem difficulty.
Brief Psychiatric Rating Scale (BPRS)
The BPRS, a clinician-based rating scale for use with adults, emphasizes significant psychiatric symptoms.
Brief Psychiatric Rating Scale for Children (BPRS-C)
The BPRS-C is a 21-item, clinician-based rating scale designed for evaluating psychiatric problems of children and adolescents.
Brief Symptom Inventory (BSI)
The BSI is a 53-item, self-report symptom inventory designed to reflect the psychological systems of psychiatric, medical, and normal individuals.
Child and Adolescent Functional Assessment Scale (CAFAS) / Preschool and Early Childhood Functional Assessment (PECFAS)
The CAFAS assesses functional impairment in children and adolescents with emotional, behavioral, or substance use disorders. The PECFAS is the version for children 4-7 years of age.
Child and Adolescent Needs and Strengths (CANS)
The CANS is a clinician-based rating scale that is required for Members under age 21.
Connors' Rating Scales-R (CRS-R)
The CRS-R helps assess attention deficit/hyperactivity disorder (ADHD) and evaluate problem behavior in children and adolescents. Data is collected through adolescent, teacher, and parent self-report.
Consumer Recovery Outcomes System (CROS)
The CROS is a series of questionnaires that gather input from three perspectives: consumers of mental health services with diagnoses of severe mental illness; staff members; and very important people in the consumers' lives.
Current Evaluation of Risk and Functioning-Revised (CERF-R)
The CERF-R is administered with a client's treatment team and is for use with adults with serious and persistent mental illness, especially for those who are chronically involved with public-sector mental health services.
Globa Appraisal of Individual Needs (GAIN)
The GAIN is a progressive and integrated series of measures and computer applications designed to support a number of treatment practices.
Methadone Treatment Quality Assurance System (MTQAS)
The use of MTQAS is mandated by the Commonwealth of Massachusetts for methadone programs. The primary purpose is to track large system outcomes; however, data can be used at the provider level.
Patient Health Questionnaire (PHQ)
The PHQ is a short, clinician-based rating scale that emphasizes significant psychiatric symptoms.
Personal Experience Inventory (PEI)
This self-report inventory documents the onset, nature, degree, and duration of chemical involvement in 12-18 year-olds.
Personal Experience Inventory for Adults (PEI-A)
This self-report inventory provides information about substance abuse patterns in adults while also assessing a range of psychosocial problems associated with substance abuse.
Quality of Life Inventory (QOLI)
The QOLI is a 32-item assessment that provides a life satisfaction and outcome score based on 16 areas of life including love, work and recreation.
Medical Outcomes Study Short-Form 36-Item Health Survey (SF-36), SF-8, SF-12
The SF-12 and SF-8 represent shorter versions of the original SF-36. The SF-36 is a generic health survey that provides a profile of functional health and well-being scores, physical and mental health summary measures, and a preference-based health utility index.
Stages of Change Readiness and Treatment Eagerness Scale (SOCRATES)
The SOCRATES is a 19-item rating scale that is designed to assess readiness for change in alcohol abusers (version 8A) and drug abusers (version 8D).
Symptom Checklist-90-R
This tool is a self-report inventory that screens for nine symptoms of psychopathology and provides three global distress indicators.
Treatment Outcome Package (TOP)/Treatment Outcome Package - Substance Abuse (TOP-SA)
The TOP provides baseline comparison data and the ability to compare client change over treatment. The TOP offers individual and aggregate outcome reports, which are supplied within 30 minutes of submission. The instrument is appropriate for all levels of care and for children, adolescents, and adults. The TOP-SA is designed for use with patients with substance abuse or dependence issues. The TOP-SA can be used with adolescents and adults.
Youth Outcome Questionnaire (YOQ)
The YOQ is a 64-item parent report measure of treatment progress for children and adolescents (ages 4-17) receiving mental health services.
