HNE BH Care Management Services
HNE Be Healthy's care management system aims to expand access to services and promote the
integration of behavioral and primary care for Members with mental illness
and/or substance abuse problems. The flexible system targets the individual
needs of Members and provides them with one point of contact into the health
care system.
The system features a continuum of care management services, comprised of:
Assessment Unit provides one point of access
for referrals to care management services. The Assessment Unit evaluates the
Member's condition and makes recommendations to the most appropriate level of
care management. To reach the Assessment Unit, call 1-800-495-0086, Ext. 5633.
Targeted Outreach provides short-term
interventions for Members who have mental illness or psychosocial issues that
may interfere with their ability to access medical treatment and
community-based services. Targeted Outreach aims to remove any barriers to care
and connect Members with available resources and services, such as temporary
transportation to and from the physician's office.
Care Coordination provides supportive
services for Members who are having difficulty transitioning from
hospitalization to community-based services. Care Coordination reviews the care
Members are currently receiving to ensure that it is the most appropriate and
coordinates treatment services to support their recovery. Care Coordination
also monitors the provision of the Member's medical care. This includes
ensuring treatment compliance with any chronic conditions and developing a
crisis prevention plan with the Member to reduce further hospitalizations.
Intensive Clinical Management (ICM) is
set up for the most vulnerable Members, who are at the highest risk of
hospitalization. ICM coordinates services across all levels of care. Care
managers work with the treatment team and Members to develop an individualized
care plan to develop strategies and goals for integrating behavioral health and
medical care, preventing crisis, and ensuring functioning within the community.
ICM also provides support for pregnant women with alcohol and/or substance
abuse problems.
All of these levels of Care Management have access to:
Community Support Program (CSP) is available to any Member who is
enrolled in care management services at HNE BH. The program is composed of
community-based agencies, which provide outreach and wraparound services to
Members. To improve their health status, CSP care managers work closely with
Members to support their care plans and link them to local healthcare and
support resources.
