CFFC staff
will work with the family, the child, and the child’s
Care Planning Team to develop a unique plan tailored
to the child’s strengths and needs. The child’s
plan will address his or her needs in every part of
life including safety, health, family, friends, school,
and other social interactions. To help address these
needs, CFFC services may include some or all of the
following:
| •
|
A plan that coordinates services for the
child, across multiple agencies and providers. |
| • |
Crisis prevention and crisis plans; 24/7
crisis support. |
| •
|
Access to treatment services, such as family
therapy, in-home and out-of-home respite care, and medication
management. |
| • |
Connections to recreational, social, and
other resources in the community. |
| •
|
Education and support for parents on topics
such as medications for mental illness, mental health
issues and diagnoses, and the developmental needs of children. |
| • |
Unique supports for the child at home,
school, or in the community. |
