How do I refer a child?
Referrals to the CFFC program are accepted from any source, including families, family advocates, state agency representatives, schools, physicians, community organizations, and the faith community.

Referrals are made to the CFFC provider. If the parent or guardian calls the CFFC provider, the CFFC provider will set up a meeting for an eligibility assessment – either at the CFFC provider’s office, the parent/guardian’s home, or some other location, as requested by the parent/guardian. If someone else makes the referral call for the family, the CFFC provider will call the parent/guardian directly to discuss the referral, before setting up the eligibility assessment meeting.

Who is eligible for CFFC?
A child may be eligible if he or she has a serious emotional disturbance that significantly affects functioning at home, school, or the community. The child must also be:
Between the ages of 3 to 18 years old (the child can be up to age 22 if he or she is also receiving special education);
At risk of psychiatric hospitalization or residential care;
A MassHealth member and enrolled or eligible to be enrolled at the Massachusetts Behavioral Health Partnership; and
A resident of Brockton, Lawrence, New Bedford, Springfield, or Worcester.
Other eligibility criteria apply and will be explained by CFFC staff.

What happens if my child applies and he or she is not found eligible?
If your child is not eligible for CFFC, the CFFC provider will work with you to help you find other services and resources.

If my child is enrolled, how long will he or she receive services?
CFFC staff will conduct regular evaluations to ensure that your child is making progress and that CFFC services are still needed.

Your child will graduate or be discharged from the program when one of the following happens:
When your child’s Care Planning Team agrees that he/she has achieved the Individual Care Plan goals;
Your child is doing better and can be helped by less intense services;
Your child is moved to a placement that is expected to continue for more than 30 days;
Your child is no longer a managed care eligible MassHealth member or has permanently moved out-of-state;
You or your child’s guardian withdraws consent for your child to participate in CFFC, or you decide not to participate in your child’s care planning;
Your child reaches age 19 (or age 22, if receiving special education services);
Your child no longer lives in one of the five CFFC communities.

What if I don’t live in one of the five CFFC communities?
If you do not live in one of the CFFC communities, your child will not be eligible for CFFC at this time. However, CFFC is a pilot project, and it is possible that the program may expand to include additional communities.

If my child is enrolled, what kind of services will I receive?
CFFC provides family and children with a range of care management and support services to respond to the multiple needs identified by families.
