Coordinated Family-Focused
Care (CFFC) provides individualized, coordinated care
to children and adolescents who have serious emotional
disturbances and are at risk for out-of-home placements.
CFFC builds on family strengths and available support
systems to help children remain and function in the
community.

community setting, as appropriate; |

Reduce the use of inpatient services and/or
long-term residential services, as appropriate; |

Increase school attendance and performance; |

Improve the child’s functioning; |

Reduce involvement with the juvenile justice

Achieve parent and youth satisfaction with
